Wednesday, November 28, 2018 HCMC Classrooms 2 & 3 7 a.m. to 11 a.m. Our Chronic Disease Management Program offers screening including total lipid panel, blood sugar, blood pressure, BMI  and A1c.  if a person has a chronic condition , smokes or is overweight, they can join the program for free. Participants are screened every three months and receive education about their chronic conditions.   Anyone can show up the day of the screening to join the program.  All patients must be fasting for 8 hours to participate in the screening. Henry County Medical Center will be on hand to offer free Osteoporosis Heel Scans as well as provide information on many of our services including Lake Haven, Home Health & Hospice,  In Home Equipment and Supplies, Henry County Healthcare Center and Plumley Rehab, Wound Care, Hyperbaric Oxygen Therapy, and more.  Representatives from Eagle Creek will also be there to answer any questions veteran's may have.  HCMC will also provide give-aways at the event.
