Chronic Disease Management Screening
7:00am - 11:00am
Wednesday, February 20, 2019 HCMC Classrooms 2 & 3 7 a.m. to 11 a.m. Our Chronic Disease Management Program offers screening including total lipid panel, blood sugar, blood pressure, BMI  and A1c.  if a person has a chronic condition , smokes or is overweight, they can join the program for free. Participants are screened every three months and receive education about their chronic conditions.   Anyone can show up the day of the screening to join the program.  All patients must be fasting for 8 hours to participate in the screening. HCMC Community Educator and/or Paris and Henry County Healthcare Foundation Chronic Disease Nurse Case Manager will be available to take your blood pressure and discuss the results with you on the first and third Tuesday mornings of each month (schedule could be modified due to holidays). Senior Stars Screening

Senior Stars Screening Thursday, March 7, 2019 9 a.m. to 11 a.m. HCMC Classrooms 2 & 3 This event is for those who are age 55 and over and are members of the Senior Stars Program. Screenings include lipid panel, glucose, blood pressure, osteoporosis screening, foot care, balance screening and hearing screening. Dietitian is on hand to answer questions. The cost to join Senior Stars is $10 a year and the screenings are offered quarterly to members during the quarter of their birthday month. You must pre-register to attend because a continental breakfast is served following the screening.  An 8 hour fast (no food or drink) is required to have the screening. McKenzie Health Fair

Join us for our McKenzie Health Fair Friday, March 8, 2018 8:30 a.m. to 11 a.m. McKenzie City Hall, 2470 Cedar Street - McKenzie, TN Join us for a day of Health & Wellness - the event is free and open to the public door prizes, snacks, education, and screenings.  Screenings include blood pressures, blood sugar, cholesterol lipid panel, weight and body mass index.  Information will include physician practices, pharmacy assistance and grant services, Tennessee Donor Services, and general hospital information on our services. Carroll County Office on Aging Chronic Disease Management Screening

Wednesday, April 3, 2019 Carroll County Office on Agining ~ 625 High Street, Huntingdon, TN 9 a.m. to 11:30 a.m. HCMC and the Paris and Henry County Healthcare Foundation will be on hand at the Carroll County Office on Aging to offer a free chronic disease management screening.  Our Chronic Disease Management Program screening offers  total lipid panel, blood sugar, blood pressure, BMI  and A1c. There will be free information on our services as well as give-aways and more.
