Wednesday, September 26, 2018 HCMC Classrooms 2 & 3 7 a.m. to 11 a.m. Our Chronic Disease Management Program offers screening including total lipid panel, blood sugar, blood pressure, BMI  and A1c.  if a person has a chronic condition , smokes or is overweight, they can join the program for free. Participants are screened every three months and receive education about their chronic conditions.   Anyone can show up the day of the screening to join the program.  All patients must be fasting for 8 hours to participate in the screening. Henry County Medical Cetner Community Education will be involved with a health fair at Life Care Center of Bruceton-Hollow Rock on Tuesday, November 6 from 10 a.m. to 2 p.m.  HCMC will be offering osteoporosis screenings with information about bone health as well as other health information about the event.
