Our Chronic Disease Management Program offers free screenings including total lipid panel, blood sugar, blood pressure, BMI  and A1c.  If a person has a chronic condition , smokes or is overweight, they can join the program for free. Participants are screened every three months and receive education about their chronic conditions. All patients must be fasting for 8 hours to participate in the screening. Screenings will be done by appointment only to provide social distancing. Masks are required. Nutrition Classes geared to heart and lung health are held on the second Tuesday, every other month at Henry County Medical Center in Classrooms 2 & 3. The class is taught via telehealth by Linda Pennington, a nutritionist with Dietitian Associates, Inc. A HCMC employee will be there to facilitate the meeting. Tuesdays starting August 2nd 10am-Noon (weekly for 6 weeks) in HCMC Private Dining Room or At-Home Diabetes is a complicated disease that can strike fear, confusion, and helplessness in diagnosed people and caregivers. People with diabetes must deal not only with their disease, but also with the impact this has on their lives and emotions. A self-management approach to diabetes education gives people the knowledge, tools and confidence to take day to day responsibility of their diabetes care. It's Time to Take Charge The Paris and Henry County Healthcare Foundation, GrowWell, and Henry County Medical Center will be offering a free Take Charge of Your Diabetes Program workshop. The 6-week program is designed for people with diabetes and caregivers to learn basic skills necessary to self-manage their diabetes and work effectively with their healthcare professionals. The Take Charge of Your Diabetes Program is led by educators certified by Stanford University Diabetes Self-Management Program Master Trainers. Classes will be held every Tuesday from 10am until Noon. Cost is free and open to anyone with diabetes. Spouses, family member and caregivers are welcome to attend. A free lunch is included with each session. Spaces are limited for the 2-hour workshop. The workshop is offered by the Paris and Henry County Healthcare Foundation Delta Rural Health Initiative. Begins Monday, August 8, 2022 4 p.m. - 5 p.m. HCMC Private Dining Room Join us for the PreventT2 Diabetes Prevention Program and learn how to prevent this chronic health condition. As part of a PreventT2 group, you will work with a trained lifestyle coach and other participants to learn the skills you need to make lasting changes. You will learn to eat healthy, add physical activity to your life, manage stress, stay motivated, and solve problems that can get in the way of healthy changes. PreventT2 is a year-long program with weekly meetings for the first 6 months, then meetings once or twice a month for the second 6 months to help you maintain your healthy lifestyle changes. By meeting with others who have pre-diabetes you can celebrate each other’s successes and work together to overcome obstacles.
