Tuesday, January 21, 2020 Henry County Office on Aging 8:30 a.m. - 10:30 a.m. Our Chronic Disease Management Program offers screening including total lipid panel, blood sugar, blood pressure, BMI  and A1c. If a person has a chronic condition , smokes or is overweight, they can join the program for free. Participants are screened every three months and receive education about their chronic conditions.   Anyone can show up the day of the screening to join the program.  All patients must be fasting for 8 hours to participate in the screening. Results will be available immediately. All patients must be fasting for 6 hours to participate in the screening. Screening will be held at Henry County Farm Bureau on 406 N Poplar Street in Paris. Start by attending our Heart Health Event to recognize Heart Health Month. Get a carotid artery ultrasound as well as your blood pressure, full lipid panel cholesterol, and blood glucose by appointment only at our Heart Strong Heart Health Fair. When calling to register, please be prepared to provide your name, phone number and date of birth. Limited appointments available and you must not have had the carotid artery screening performed within the past 5 years. Goodie bags will also be given.
