W-2 FORM PICK-UP SCHEDULE

If you are a FORMER partner of HCMC or HCHC, your W-2 Form is available for PICK-UP.

All W-2 Forms are also available on PAYCOM located under PAYROLL YEAR END TAX FORMS.If you did not elect to receive electronic forms only, a hard copy will be available for PICK-UP.

Important PICK-UP Details:

You MUST bring your ID Badge or a photo ID.
You may only pick up your own W-2.
You CANNOT pick up for another partner.

Any W-2 form not picked up will be placed in outgoing mail on Tuesday 01/30/2024.

*W-2 forms will be brought to location for staff to pick up. All other times all W-2’s may be picked up in the Accounting Department.
