PARIS, TN – After being named Top 25 National Best Practice Agency for Hospice earlier this month, Henry County Home Health and Hospice received notice that they have been named a Top 25 National Best Practice Agency by Fazzi Associates for Home Health.

Based on overall survey results for the year 2020, Henry County Medical Center Home Health and Hospice’s Home Health Department received the 2021 Award of Distinction for demonstrating superior performance in patient satisfaction.

“Agencies named as national best practice agencies are proven leaders and have excelled in one of the most important measures of an agency’s quality program– caregiver experience,” said Jennifer Ittner, Project Director for Fazzi Associates. “This is a significant accomplishment that you, your employees, and board should be incredibly proud of.”

Based on a comparative analysis of agencies involved in Fazzi Associates’ Home Health CAHPS system for the past year, the results for the Overall Satisfaction or Likelihood of Recommending survey questions placed HCMC Home Health in the top 25% of Fazzi’s National Home Health CAHPS database.

“This award is presented to home health organizations that have demonstrated superior performance in 2020,” according to Ittner.

Home Health is designed to provide the smoothest transition from hospital to home. Patients must be referred to Home Health and must be homebound, need skilled care in the home and be under the care of a physician.
