    Our Chronic Disease Management Program offers free screenings including total lipid panel, blood sugar, blood pressure, BMI  and A1c.  If a person has a chronic condition , smokes or is overweight, they can join the program for free. Participants are screened every three months and receive education about their chronic conditions. All patients must be fasting for 8 hours to participate in the screening. Screenings will be done by appointment only to provide social distancing. Masks are required. Screening will be held at HCMC Diagnostic Center Atrium. For more information on these screenings, to make an appointment, or to join our Chronic Disease Management program, call 731-644-8215 or email tumstead@hcmc-tn.org.

    9:00am - 10:00am
    HCMC Community Educator and/or Paris and Henry County Healthcare Foundation Chronic Disease Nurse Case Manager will be available to take your blood pressure and discuss the results with you on the first and third Tuesday mornings of each month (schedule could be modified due to holidays).     Ever wondered if you are selecting the best food choices for your lung disease?

    Henry County Medical Center’s Pulmonary Rehab as well as Food & Nutrition Services is offering you the opportunity to learn what to eat for a healthy heart thanks to the Paris-Henry County Healthcare Foundation. The class is limited to 24 participants and Pulmonary Rehab patients have first priority to attend the class.  Please, we ask you leave children at home.  The cost for this class is free.     Cardiac Rehab Nutrition Class

    Prevent Type 2 Diabetes Prevention Program Informational Session

    1 in 3 Americans has pre-diabetes and most do not know it. YOU MAY HAVE PRE-DIABETES and be at risk for type 2 diabetes if you:
    • Are 45 years of age or older
    • Are overweight
    • Have a family history of type 2 diabetes
    • Are physically active fewer than 3 times per week
    • Ever had diabetes while pregnant (gestational diabetes) or gave birth to a baby that weighed more than 9 pounds
    Join Us for an Informational Session about the Program! Monday, May 24, 2021 4 p.m. - 5 p.m. HCMC Private Dining Room Join us for the PreventT2 Diabetes Prevention Program and learn how to prevent this chronic health condition.  Participants who attend the informational session will receive a free A1c screening at the event. As part of a PreventT2 group, you will work with a trained lifestyle coach and other participants to learn the skills you need to make lasting changes. You will learn to eat healthy, add physical activity to your life, manage stress, stay motivated, and solve problems that can get in the way of healthy changes. PreventT2 is a year-long program with weekly meetings for the first 6 months, then meetings once or twice a month for the second 6 months to help you maintain your healthy lifestyle changes. By meeting with others who have pre-diabetes you can celebrate each other’s successes and work together to overcome obstacles Call the HCMC FindLine at 731-644-3463 to join.
