That is why we are thrilled to have Transitions Health available to patients 18 years and older in our outpatient clinic as well as in Plumley Rehab and Eiffel Gardens.
For those individuals recovering from a hospital stay, Transitions Health can greatly simplify the switch from hospital to home. And for the residents of Henry County Healthcare Center who view HCHC as their home, Transitions Health plays a critical role in allowing these residents to remain in their normal environment while receiving a more intense level of care- helping to relieve stress both for the residents as well as their family members.
