Home Health and Hospice

 

HCMC Angels of Light angel graphicHome Health and Hospice is designed to provide the smoothest transition from hospital to home. Patients must be referred to Home Health and must be home bound, need skilled care in the home and be under the care of a physician.

Hospice is support for people with limited life expectancy and their families. We provide a comfortable, supportive environment, ensuring that the quality of life is the best it can be. We pay special attention to physical, spiritual and psychological needs of our patients and their families.

There is an RN on call 24 hours-a-day taking care of doctor’s orders. We have social worker counseling for patients and families and also a medical director for consults with physicians and pharmacists for orders on pain control, physical, speech, occupational and dietary counseling.

There are volunteer services for support, companionship, personal care and to run errands. Ask for Carman Wright, MSW, if you would like to train as a volunteer.

Our hospice chaplain can help with spiritual counseling and there is a grief support group meeting monthly. Home health services provided include:

  • Skilled nursing care.
  • IV therapy.
  • Chemotherapy.
  • Pediatric and maternity care.
  • Physical therapy.
  • Occupational therapy.
  • Speech therapy.
  • Home health aides.
  • Medical social services.
  • Laboratory services.
  • Medical supplies.
  • Wound care.
  • Patient and caregiver teaching.
  • Injections.
  • Central line maintenance.
  • Total parenteral nutrition.
