At Henry County Medical Center, we believe that every patient should have easy, instant access to his or her health information at any time. We are pleased to offer this opportunity through the use of a secure internet portal. Using this portal, you will be able to access your health information online.
All you need to activate this service is a valid email address. You will then be able to access information from your visit or hospital stay including allergies, procedures, lab and radiology results, vitals, and more—all in one place!
Getting access to your medical records is a very simple process that begins when you register at HCMC (see below for Step by Step details). During the registration process, you will be asked to provide a valid email address, which upon discharge from the hospital, you will receive an email with a link to Patient Portal. Open the email and click to launch the online Patient Portal, where you will be prompted to enter you date of birth to confirm your identity.
Once your identity has been confirmed, you will be prompted to verify your demographic information, create a User ID, and add security questions. Once you have done this simple process, your online Patient Portal will be complete and ready to access your health information in a secure format.

During the Registration process, be sure to provide your registration clerk with a valid e-mail address so that you receive the information you will need to access the health information portal.
Upon discharge from the hospital, you will receive an e-mail with a link to the Health Information Patient Portal.
Check your e-mail and use the link to launch the online Patient Portal.
You will be prompted to enter your date of birth to confirm your identity.
You will be prompted to verify your demographic information including your name, date of birth, gender, e-mail address, and zip code.
Create a User ID (using your e-mail address is recommended) and a password.
Create three security questions to verify your identity in case of password loss.
Congratulations! Your account has now been activated!
Once you have established your account, you can log in here: https://app.relayhealth.com. This should only be used after you have completed your initial sign-up/login to the Health Information Portal.
